Make a list of the events which have had a negative impact on you. Put the list in order from prenatal until present day. Make sure to include the following traumas:
Prenatal Injuries - ultrasounds, x-rays, drugs taken by the mother while pregnant, accidents experienced by the mother, and emotional traumas experienced by the mother
Birth - being born in itself is the first physical shock to the body, include any birth complications, epidurals, etc.
Vaccinations - childhood vaccines, flu vaccines, rabies vaccines, vaccines for travelling, etc.
Drugs - recreational drugs, prescription drugs, antibiotics, anaesthetics, antidepressants, etc.
Physical Injuries - surgeries, dental proceedures, accidents, broken bones, head injuries, etc.
Emotional Traumas - abuse, death of a loved one, divorce, witness to a crime or accident, etc. Include anything that caused you to feel grief, anger, jealousy, fear, guilt or shame.
Other - exposure to x-rays, radiation, toxic chemicals, etc.
If you are not sure about an event, write it down anyway. If you feel uncomfortable with sharing the specifics of a trauma just write down the result of what happened. For example "I experienced an event in which the result was a broken bone and the emotion fear." Don't worry if you forget something because it can always be added in later.
